All applications received by the Commission must include the information described in the Regulations and application forms.

Applicants who satisfy each of the Commission’s requirements under sections 34 et seq. of the Regulations may be issued a Client Provider Authorization. Please review the provisions of the Regulations - or contact the Commission office - for further details.
